Our Supported Independent Living homes are designed for NDIS participants, so you can live independently in shared accommodation. Unlike other NDIS providers with CRT Program you will only be sharing with one other person. Supports are 24/7 and can assist each resident with their daily life at home including:
Cooking, meal preparation
and nutrition
Cleaning and daily
household tasks
Personal care & assistance
with medication
Our shared accommodation provides opportunities for great support and companionship and to be part of your local community. The shared living accommodation we offer is modern, state of the art and flexible to adapt as your needs change. Together, we’ll work with you through every step, as you progress towards your new shared living arrangement, and ensure your home reflects who you are and the support you need.
We atCRT program able to assist in providing the proof of permanent accommodation solutions while you are in transition process.
Before moving into your new home, we get to know what is important to you.
Some of the things to think about include:
● Where your will home be
● Who you will live with
● Identify the supports, medical assistance and allied health needs you have
● Any personal preferences you have to make your staff comfortable and fun!
● How we can best work with you to prepare you for the move
● We will agree on a plan that works for everyone.
There are many practical things to think about during this stage, and we want to make sure this is an enjoyable experience. Some things we consider and plan together include:
● Understand what kind of staff are you looking for or if you want to bring your own staff with you
● What skills and shared interests you would like them to have
● Any equipment, assistive technology or home modifications required
● Any furniture that needs to be provided
Once you’ve moved in, our time is spent getting to know you even better. Once we’ve met your goals for living arrangements, we want to understand more about what you need and desire to be able to live your best life. We will continue to review, discuss and check-in with you and your support networks.
